Medication & Surgery for Glaucoma

Glaucoma symptoms can be reduced and intraocular pressure can be decreased with certain medications. Eye doctors will usually try drug therapies first for glaucoma patients rather than resorting to surgical techniques. 

Some of the medications we recommend for glaucoma include:

  • Beta-blockers to reduce fluid production
  • Carbonic anhydrase inhibitors to reduce fluid production
  • Miotic or cholinergic agents to increase fluid outflow
  • Prostaglandin-like compounds to increase fluid outflow
  • Alpha-agonists to reduce fluid production and increase drainage.
  • Epinephrine compounds to increase fluid outflow.

If intraocular pressure in your eyes is high and you need immediate relief, we may recommend surgery. There are 2 main surgical procedures used to drain fluid from the eye for those with glaucoma:

  • Laser surgery: During laser surgery for glaucoma, a high-intensity laser is used to help open the eye’s trabecular meshwork—its natural drainage system—and allow fluid to drain out of the eye. 
  • Filtering microsurgery: During filtering microsurgery, small incisions are made in the eye to help fluid drain naturally. This procedure is often reserved for emergencies or used when laser or medication treatments have not been effective.

Selective Laser Trabeculoplasty (SLT)

Selective laser trabeculoplasty (SLT) is used in the treatment of glaucoma and can be an important tool in lowering intraocular pressure. This treatment can use automated technology to increase efficiency and promote positive outcomes.

The Ex-Press mini-shunt is inserted under the conjunctiva of the eye to decrease intraocular pressure. The conjunctiva of the eye is the thin membrane that covers the eye and protects it. This shunt is made of metal and is quite small (about 3 millimeters long).

The Xen gel stent is a surgical implant meant to lower the intraocular pressure for those with open-angle glaucoma. It is used as a supplement to other glaucoma medication treatments or when previous surgical procedures have not been effective.

The stent is a small flexible tube that is placed under the conjunctiva.

The iStent Infinite is an implantable device that is used to lower intraocular pressure. It is used where previous surgical procedures have failed to treat open-angle glaucoma and can also be used for patients undergoing cataract surgery.

This device provides 24/7 intraocular pressure control and is considered microinvasive.

Peripheral iridotomy is a laser procedure where the laser creates a hole in the iris to help relieve intraocular pressure by allowing fluid to escape. Peripheral iridotomy often uses the argon laser.

DURYSTA is an innovative, FDA-approved treatment for glaucoma and ocular hypertension that offers a consistent option for lowering intraocular pressure (IOP).

DURYSTA is a tiny, biodegradable implant that is placed inside the eye to deliver a consistent dose of medication directly where it’s needed most.
